This collection consists of indexes and images of license applications to practice dentistry in Washington. Includes statements from practicing dentists in 1888 that were required by the Territorial Board of Dental Examiners. Information on these forms includes name, age, birthplace, residence, place of practice, locations of previous practice, as well as, educational background.

Between 1909-1920 Applications for Examination sent to the State Board of Dental Examiners included name, residence, birthplace, age, date of application, prior places of practice, and educational background.

Applications from 1921-1936 submitted to the Department of Licensing include name, residence, birthplace and date, citizenship status, previous places of practice, and educational background.

Prior to 1921, the Board of Dental Examiners oversaw the professional licensing of dentists. The Department of Licenses was established to bring together the independent licensing boards for various professions. The Department also became responsible for granting registration and licenses for cars and their drivers. In 1989 the Department of Health assumed the responsibility for the licensing of dentists.
